What should I bring with me to my MMJ evaluation?
If you're under the age of 30 or require an extended plant count (6+ plants), you must have proof of legible medical records proving your ailments that qualify you for a cannabis prescription. These may include, but are not limited to:
- Hospital Discharge Summaries
- Hospital histories and Physicals
- Lab Work
- Doctor office notes
- Imaging and X-ray Reports
- Surgical Reports
- Anything else deemed significant to your medical condition
All patients, regardless of age or desired plant count, need to bring a valid form of ID, such as a Colorado State Driver's License or State ID.
When applying with an out of state ID, two forms of residency (W2, paystub, Government issued benefit letter, certified Colorado tax return, or Colorado motor vehicle registration) must also be presented. Two forms of ID are also required (Passport, Military ID, Social Security Card, Tribal ID, Social Services Card). Renewing patients, however, are required to have a Colorado State ID.
What ailments qualify me for medicinal marijuana?
Colorado Medical Marijuana Law states that qualifying conditions for prescription cannabis are as follows:
- Cachexia
- Cancer
- Chronic pain
- Chronic nervous system disorders
- Glaucoma
- HIV or AIDS
- Nausea
- Persistent Muscle Spasms
- Seizures
- PTSD
It should be noted, however, that each case will be evaluated individually and decisions will be made at the discretion of each medical professional.
What are patient possession limits for medicinal marijuana?
Colorado Medical Marijuana Law notes that two ounces of usable marijuana are allowed per patient.
If a patient is approved for an extended plant count, however, these possession limits may be increased at the doctor's discretion.
Are there any limits to home cultivation of medicinal marijuana?
Patients (or primary caregivers) may cultivate no more than six marijuana plants with three or fewer being mature, flowering plants that are producing a usable form of marijuana. Though there are cases where more than six plants are allowed this is usually only prescribed for patients with severe medical conditions, such as cancer.
What is the cost of an appointment?
Depending on your desired plant count the cost of an appointment will vary:
- Standard Plant Count (6 plants), First time applicant/Renewal: $125 ($95 w/ coupon)
- Extended Plant Count (7-24 plants): $335 ($250 w/ coupon)
- Extended Plant Count (24-99 plants): $600 ($450 w/ coupon)
What happens after my medical evaluation?
After your appointment (assuming you qualify for medicinal marijuana), we will help you complete an online application for a medical marijuana card to be submitted to the state. This service is offered at no additional charge.
This includes:
- Creating an online account with the state.
- Adding your patient registration to your newly created state account.
- Uploading your "Physician Certification" to your state account.
- Guiding you through the state payment portal. (The state asks for $16.10 to process your card. This payment must be made with a credit card, debit card or electronic check.)
- Providing you with copies of your application and physician certification for your records and for temporary card purposes, if applicable.
Is it possible to submit my medical card application online?
As of January 1, 2017 the CDPHE began accepting online applications. As described above, our qualified staff will walk you through the online application process which will speed the delivery of your medical card, ensuring that you receive it within 48 business hours.
How do I renew my card?
You must submit a renewal application every year to remain on the State registry. The renewal process is identical to the new patient application process.
How do I check on the status of my medical marijuana card?
